If someone says stop (barring it being some sort of bizarre fetish film) then the other person should stop having sex with them. Continuing is sexual assault.
It's not uncommon for the directors to hide scenes until the people arrive on set, or even right before the segment gets filmed. It's almost always things the person said they didn't want to do, but once they're on set it's easy to blackmail them into it. I have empathy here since the ways young people get brought into porn are skeezy as hell, and play on their normally messed up backgrounds. The industry destroys people regardless of their gender.
That's not to say they don't bear any responsibility, just that the actors are at a major power disadvantage.
